4. Instructions on the procedure for merging the latest social insurance books:
Pursuant to Clause 4, Article 46 of Decision 595/QD-BHXH stipulates as follows: “If a person has 2 or more social insurance books with different periods of payment of social insurance, the social insurance agency shall withdraw all social insurance books and refund them. re-edit the database, print the time of payment and enjoyment of social insurance and unemployment insurance premiums of the social insurance books into the new book.”
Participants with 2 or more social insurance books need to carry out procedures to combine the books into a single social insurance number so that the social insurance agency can conveniently record the process of paying and enjoying social insurance.

Step 1: Check personal information on social insurance book
Checking the participant’s personal information (full name, date of birth, gender, ethnicity, nationality) on 2 social insurance books, there are 2 cases as follows:
Case 1: The employee’s personal information matches => continue to check the social insurance payment process
Case 2: Personal information of participants is different => make a profile to adjust personal information so that the information on the 2 social insurance books matches.
Records include:
– Form TK1-TS (Declaration of participation, adjustment of information on social insurance and health insurance): The unit clearly states the request to include the social insurance book in item number (14) content of changes and requests.
– Social insurance book wrong information
– Documents (ID card/birth certificate/birth extract/…)
