The list of information is made by the employer in the organization or enterprise. This is a statement that provides data, facts and information related to employees. Model D01-TS Issued together with Decision No: 595/QD-BHXH dated April 14, 2017 of Vietnam Social Security.

1. Meaning of making a list of social insurance information:

The list of information is made in the management activities of the Social Insurance agency. In it, to provide necessary information about related papers, records, documents or documents. This makes sense in the general work as well as the actual use purpose.

Form D01-TS according to Decision 595/BHXH – A list of information is prepared for the following purposes:

– Summarize dossiers and papers of units and participants as a basis for arrears of social insurance, health insurance, unemployment insurance, occupational accident and occupational disease. In which, relevant information is aggregated into different columns set up in the table. Papers, documents and information are aggregated in the sense of listing information. Assist in finding, using and exploiting such data.

– Re-issuance, change, adjustment of contents recorded in the social insurance book, health insurance card enclosed with the list of employees participating in social insurance, health insurance, unemployment insurance, occupational accident and occupational disease (Form D02-TS) or declaration of participation in social insurance and health insurance (Form D02-TS) Model TK1-TS). Implementation in actual arising needs to use the information already available. The retention of information is very important in the need to change, modify or use the information provided. Especially when the insurance agency needs to have complete and accurate information about the subjects involved.

– Responsibility to establish: The employer will be responsible for preparing the form D01-TS. To summarize information related to labor of the enterprise. Help ensure the rights and obligations of each subject in accordance with the law. The employer needs to update to bring accurate and effective information related to the declaration in the table.

– Setting up time: When there is a problem for updating employee information.

– Basis for making form D01-TS: Papers according to Section 2, Appendix 01; Appendix 02; Appendix 03. Specify the original/copies/authenticated copies of the papers. To ensure that the information provided is correct. Since then, the use and listing of the information in the table makes sense for future use.

3. Basis for making form D01-TS List of information:

4. Instructions for setting up List of information (form D01-TS):

In the content of the list, the contents should be presented in columns and rows. Shows the source of information to be provided related to the base, employees, etc.

Setting method:

* Horizontal indicators:

– Indicator (1): write the contents of the list. For example, a dossier used as a basis for arrears of social insurance, health insurance, unemployment insurance, occupational accident and occupational disease insurance or a dossier used as a basis for adjusting information on participation in social insurance, health insurance, unemployment insurance, occupational accident and occupational disease insurance. Thereby identifying the purposes to search for relevant information sources effectively. These needs are associated with the respective interests of employees when participating in Insurance. And they need to be provided as well as reflect relevant information in adjusting or updating information.

– Indicator (2): write the attached list. For example, attach a list of employees participating in social insurance, health insurance, unemployment insurance, occupational accident and occupational disease insurance (Form D02-TS). Or enclosed with the declaration of participation in social insurance and health insurance (Form TK1-TS). The attached documents are intended to identify and compare the employee’s information. As well as ensuring effective management of employees participating in Insurance in the unit.

The indicators in horizontal rows are presented in association with the purpose of establishing the list of the employer. Thereby providing corresponding information for each employee participating in the enterprise’s Insurance.

* Criteria by column:

– Column 1: write the ordinal number.

– Column 2: write the full name of the adjustment participant.

– Column 3: write the adjustment participant’s social insurance number.

– Column 4: write the name, type of document (Decision, Labor Contract, Certification …).

– Column 5: write the document number (99/QD-UBND, 88/LDTBXH-NCC …).

– Column 6: write the date of issue of the document.

– Column 7: write the effective date of the document.

– Column 8: write the agency issuing the document (the People’s Committee of the district, province or Department, branch …; Company A …).

– Column 9: write the content of the text extract (recruitment, mobilization, salary increase; confirmation of people with meritorious services to the revolution…).

The contents of these 9 columns are normally recorded for information transmission purposes. In column 10, pay attention to the information in the purpose, based on the specific consideration. Thereby providing the most necessary information for information enumeration needs.

– Column 10: write a number of information summarized in the papers so that the social insurance agency has grounds for appraisal such as:

+ Collection: Write some contents in the document as a basis for arrears. Those are the contents, the determining grounds are used as bases. As well as providing specific content what is the basis of use so that readers can understand without looking up other documents.

+ In case of adjustment of contents recorded in the social insurance book. Such as adjusting to doing heavy, hazardous or particularly heavy, hazardous occupations or jobs on the list promulgated by the Ministry of Labor, War Invalids and Social Affairs and the Ministry of Health. The nature of these jobs should be provided with relevant information. To determine in the employee’s benefits received in the operation of the enterprise. Which includes:

+ Specify the job, working location.

+ Salary, salary allowances, additional amounts or salary levels, salary coefficients, time of salary enjoyment of employees.

This information must be provided accurately according to the Decision on assignment of occupations or jobs or the Decision on salary or labor contract, labor contract according to profession or job.

+ Re-issuance of social insurance books due to changes in employee’s relevant information:

Such as changing last name, first name, middle name; date of birth; sex; nationality. The following information should be provided upon request:

Full name; date of birth; sex; the participant’s nationality is stated in the Birth Certificate or Birth Extraction;

Specify: ID card/ID card/passport number; Full name, date of birth of the participant as stated in the ID card/ID card/passport.

If you are a party member, please specify: full name; date of birth; The date of the participant’s profile declaration is recorded in the Party member’s curriculum vitae. + In case of enjoying higher health insurance benefits:

For people with meritorious services to the revolution who are issued with invalids card, sick soldier card, certificate of policy beneficiaries as war invalids:

+ Specify the full name, date of birth, rate of loss of labor of the person with meritorious services to the revolution written in the card.

+ Full name and position of the person signing the card.

For people with meritorious services to the revolution who are granted decisions on recognition, decisions on allowances, certifications, certificates, orders, medals, etc. (for abbreviated documents):

+ Specify the full name and date of birth of the person with meritorious services to the revolution stated in the document (if any);

+ Full name and position of the person signing the document.

For veterans under Decree 150/2006/ND-CP dated 12/12/2006 of the Government (Decree No. 157/2016/ND-CP dated 24/11/2016 of the Government amending and supplementing Decree No. Decree No. 150/2006/ND-CP):

+ Specify the Decision’s name (being a member of staff, being discharged from the army, changing majors);

+ Date of enlistment; military rank (standard lieutenant, second lieutenant…);

+ The location of the veteran’s station is stated in the document;

+ Full name, rank of the person signing the document (or signing for document appraisal).

For veterans who are directly involved in the resistance war and are granted Certificates, Certificates of Merit, Decisions on allowances, CVs (cadres, party members):

+ Specify the full name, date of birth of the veteran mentioned in writing;

+ Full name and position of the person signing the document.

For those who enjoy higher benefits according to the household. For example: relatives of people with meritorious services to the revolution, poor households… are granted certificates, certifications, household registration books, temporary residence books:

+ Specify the full name of the person with meritorious services to the revolution (or the head of the household).

+ Full names of relatives are recorded in the document.

+ Full name and position of the person signing the document.

Thereby providing accurate relevant information to prove the interests of the subject. Ensure effective verification and identification of respective subjects and interests. Thereby receiving the right insurance regimes in accordance with the law. Bring the efficiency of information provision of the employer as well as the efficiency of the insurance agency.

* Note: In case the participant does not have the documents mentioned in Appendix 02, Section II, III of Appendix 03 but has other documents to prove it, the unit shall submit it to the social insurance agency for consideration and settlement, but not in this list.
